The student(s) named on the front side of this form wishes to enroll & participate in muay thai training & activities conducted by Garden Grove MT, whom is owned by Garden Grove Muay Thai, LLC and affiliates. In consideration of permitting the student, the student (acting through parent/guardian, if a minor) agrees as follows:

1.  The student agrees to abide by all rules governing the conduct & operation of instruction. Respect for other students & observers, promotion tests & similar activities as prescribed by Garden Grove MT.
2.  In consideration of allowing the student to enroll & participate in muay thai training & activities at Garden Grove MT, the student hereby voluntarily releases, discharges, waives, & relinquishes any & all causes of action/claims for personal injury, property damage, or wrongful death occurring to the student as a result of engaging or receiving instructions in said activities related there to, wherever/however the same may occur & for whatever period such activities & instruction may continue. The student & parent(s)/guardian(s) makes such release & waiver on behalf of the student, student heirs, executers, administrators, & assigns to prosecute/present any claims for personal injury, property damage, or wrongful death against Garden Grove MT, any employee, agent, or affiliate of Garden Grove MT, or any other instructor providing martial arts training or activities in connection with Garden Grove MT. For any claim/cause of action whatsoever, whether the same shall arise by negligence of any of said persons, or otherwise. It’s the intention of the student by this agreement to exempt Garden Grove MT and all persons associated with Garden Grove MT or facilities from liability of any kind of personal injury, property damage, or wrongful death caused by negligence/by accident of any person.
3.  I understand & agree to this agreement Not to Slander (State of California civil code Section 46) or make any accusations against Garden Grove Muay Thai LLC or affiliates via including, but not limited to Google, Facebook, Yelp, or any other platform, etc.
4.  The student(& the parent(s)/guardian(s) if applicable) understands that muay thai training & activities may include striking, kicking, choking, falling, & other activities which could produce such injuries/even death. Garden Grove MT & its affiliates will exercise reasonable judgement, skill & supervision to reduce likelihood of any such injuries. However, the student & or parent(s)/guardian(s) assumes & accepts such risks & agrees to the waiver & release of liability against Garden Grove MT, companies, & all persons associate with them for any injury which may occur in course of training and activities, even if such injury or death is caused by negligence.
5.  By signing this agreement I am entirely and fully responsible for any minor or child that I bring inside Garden Grove Muay Thai Facility located at 10612 W Garden Grove Blvd, Garden Grove, CA, 92843 and any events.
I, the undersigned student & or student’s parent/guardian have carefully read all the foregoing, & have had the opportunity to fully discuss any questions & concerns that I may have with the representative of Garden Grove MT, including the nature & extent of any risks/dangers associated with such martial arts activities. Having read & carefully understood all of the above I agree to its terms in full.
